- b. Is the restitution recipient an insurance company or state agency, watch out for the Crime Victim Compensation Fund?
- c. Have you Paid off the principal balance?
- ▢ Dates of incarceration (on this cause number), if applicable
- ▢ Prosecuting Attorney’s mailing address

- ▢ Cover Letter for the Court
